Our comprehensive services maximize functional abilities, promote improvement and maintenance of physical and mental health, and support a sense of control and self-determination in a nourishing environment. Our skilled nursing services are individualized under the supervision of the participant’s Primary Care Doctor.
From a nutritional program to transportation services, we help with daily living tasks.
As part of our nutritional program we provide breakfast, snacks and a hot meal daily. We offer transportation services in our new minibuses. We stay closely connected to our clients’ community case managers, visiting nurse services, families and doctors to provide best possible care.
